7. HEALTH REQUIRMENTS & PAR-Q COMPLIANCE

At DC-UNIT, we train warriors—but safety is always the first battle won.
Before participating in any physical activity at our facility, all members are required to complete a PAR-Q (Physical Activity Readiness Questionnaire). This form helps us understand your current health status and identify any conditions that may require medical clearance before engaging in exercise.
By signing up for membership or attending classes, you confirm that:
- You have completed the PAR-Q form truthfully and to the best of your knowledge.
- You have disclosed any relevant medical conditions, injuries, or limitations.
- You understand that it is your responsibility to consult with a healthcare professional if you answered “Yes” to any PAR-Q questions or have any concerns about your ability to safely participate. 

PAR-Q Form is on the booking page for all classes. 
DC-UNIT reserves the right to request medical clearance or deny participation if we believe your health status may pose a risk to yourself or others.
Your health is your foundation. Your honesty is your armor.
